    FORMS - What must be mailed by June 1st

  1. Print and mail a signed Youth Release Form

  2. All non-market animal exhibitors print and mail a 4-H Code of Ethics (beef, dairy, goat, poultry, rabbit, sheep, swine, horse, dog & cat)

  3. Overnight Exhibitors, UME Volunteers and Parents print and mail Overnight Health Form and Adult Release Form.  (Note:  if Exhibitors and UME Volunteer completed  Health Form through the 4-H online enrollment system, do not mail the form.)

  4. Livestock entry fees and payment for retail shirts mailed with a completed Fee Voucher.

  5. Campers print and mail by July 1st the Camper Registration Form

  6. Livestock & Small Pet Exhibitors print and bring Self Certification Form for review by the Vet or Department Superintendent,
                   (beef, dairy, goat, poultry, rabbit, sheep, swine, horse)
