  1. Youth Release Form Print, sign, mail

  2. Fee Voucher - mail with check for Livestock entry fees and retail shirt payment

  3. 4-H Code of Ethics  Non-market animal exhibitors, print, sign, mail (beef, dairy, goat, poultry, rabbit, sheep, swine, horse, dog & cat)

  4. Self Certification Form Livestock & Small Pet Exhibitors print, sign, bring for review by the Vet or Department Superintendent
       (beef, dairy, goat, poultry, rabbit, sheep, swine, horse)

  5.  Adult Release Form   Overnight UME Volunteers & Parents print, complete, mail

  6. Overnight Health Form Overnight Exhibitors, UME Volunteers & Parents print, complete, mail  (Note:  if completed  Health Form through the 4-H online enrollment system, do not mail.)

  7. Camper Registration Form   Print, mail by July 1st

 


NOTE – LIVESTOCK AND SMALL PET EXHIBITORS
Self-Certification Requirements

Per MDA requirements, livestock Exhibitors shall ensure animals they present at shows are in good health.  Exhibitor or hauler must complete Self Certification of Animal Health prior to loading and present upon request.

For large animals,  the MDA’s Livestock Self Certification document must be completed.   

 Self Certification must be completed on the EquinePoultry and Rabbit Certification documents.
